Evaluates children with or at risk for developmental delays throughout Utah. Provides developmental assessments and management recommendations for all ages of children and assists families in coordinating child needs. UDAC locations are in Provo and Salt Lake. Programs include Child Development Program (CDP), Heart Center Neurodevelopmental Program (HCNP), and Neonatal Follow-up Program (NFP).

For infants who meet any of the following criteria: a birth weight less than or equal to 2 lbs, 12 oz; gestational age under 26 weeks; ECMO support required; diagnosis of HIE, or referral by a medical provider.

Neonatal Follow-up is a statewide program whose threefold mission is to:
-Offer 2½ year interdisciplinary follow-up services to an at-risk graduate from a referring newborn intensive care unit (NICU)
-Complement the care provided by the child’s medical home
-Provide feedback to the referring NICU
The program offers periodic screening by a multidisciplinary team. When indicated, the program assists with or refers to appropriate intervention and financial services. The program provides psychosocial support as needed.
The NFP services are tailored to the needs of each child. A summary of each evaluation is sent to the child’s medical home and to the referring newborn intensive care unit. With parent approval, the summary is also sent to other providers involved in the child’s care.
Services offered: Developmental pediatrics, Child psychology (developmental testing), Speech/Language pathology, Nutrition (nutritional analysis), Audiology (hearing evaluation) Neurology (brain/nervous system evaluation--Provo location), Physical/Occupational therapy (motor skills evaluation).

Clinic hours two Friday mornings per month, 8:00 am to noon

The University of Utah is assuming responsibility for clinical services formerly provided by the Bureau of Child Development, Utah Department of Health.

Ages 0 - 21. Provider referral required. Accepts private pay and several insurance plans. 

Provides clinical assessments, diagnostic services, consulting and case management to any infant, child, or adolescent who may have a developmental need.

Ages 0-18 and referred by a cardiologist. Sliding scale based on income.

Provides assessment, guidance and resources for children born with complex congenital heart disease. HCNP is a longitudinal program that starts while the infant is in the hospital. Patients are seen as part of a longitudinal program starting inpatient at infancy, at 12-24 months, at 3-6 years, and 9-12 years.

High risk conditions include: cyanotic heart disease; open heart surgery as a newborn, other risk factors combined with heart disease including prematurity, ECMO, genetic syndrome, seizures, heart transplant and history of cardiac arrest.

HCNP clinics are held twice a month. Assessments can take up to 4 hours.
