Eligibility:
Ages 0-18 and referred by a cardiologist. Sliding scale based on income.
Services Provided:
Provides assessment, guidance and resources for children born with complex congenital heart disease. HCNP is a longitudinal program that starts while the infant is in the hospital. Patients are seen as part of a longitudinal program starting inpatient at infancy, at 12-24 months, at 3-6 years, and 9-12 years.
High risk conditions include: cyanotic heart disease; open heart surgery as a newborn, other risk factors combined with heart disease including prematurity, ECMO, genetic syndrome, seizures, heart transplant and history of cardiac arrest.

Provider Profiles: HCNP clinics are held twice a month. Assessments can take up to 4 hours.
