Eligibility:
Individuals who reside in Utah and are eligible for Traditional Medicaid Services, who do not have transportation to medical appointments.
Services Provided:
Provides non-emergency transportation options for eligible Medicaid members to assist with transportation to medical appointments.
UTA Bus - for those with Traditional Medicaid and who live in an area served by UTA or Cedar Area Transportation Services (CATS). Contact a Medicaid eligibility worker for more information.
Para-Transit Bus Services are available in some areas. Contact Medicaid at 801- 538-6155 or toll-free at 1-800-662-9651 for FlexTrans vouchers.
ModivCare (formerly LogistiCare) - for those with no other way to get to Medicaid covered services. Call 1-855-563-4403 three business days in advance to set up your ride. Have the following ready when calling:
- Name and birth date of the patient
- Medicaid ID
- Address and phone number of picked up location
- Name, address and phone number of Medicaid medical provider being seen
- Date and time of the appointment
- Will an attendant be riding also?
- Can the patient walk unaided? If not, what type of aid will be used?
