Individuals living with HIV disease and family members who have no insurance, are underinsured or are not eligible for a State program, assets not over $10,000 (does not include a home or one car), low income.

1. Supportive Services Program - ambulatory care, dental care, transportation services, case management, mental health services, substance abuse services, outreach services, and home health care.
2. ADAP (AIDS Drug Assistance Program) - helps eligible individuals pay for all or part of their HIV medications.
3. Health Insurance Continuation Program (COBRA) - can pay for COBRA payments for eligible individuals.

CHCs serve everyone, insured and uninsured. Patients pay for services on a sliding-scale fee structure based on their income.

Midtown Wellness Clinic, located at Weber Human Services, is designed to improve the physical health of individuals with mental illnesses and addictions. Medicare beneficiaries can qualify for a low income subsidy (also known as Extra Help) paying for their monthly premiums, annual deductibles, and co-payments related to Medicare Part D (prescription drug coverage). The Extra Help is worth about $5,000 per year.

PAF’s Co-Pay Relief (CPR) program provides direct financial assistance to qualified patients with co-payments, co- insurance or cost-sharing associated with prescription drugs through funds dedicated to specific disease states. • Prescription Drug Patient Assistance Programs

Parents of children receiving serious and financially-challenging health care treatment. Children must be 16 or younger at the time of application, have a Social Security Number issued by the Social Security Administration (TIN numbers are not accepted), must be covered by commercial/private insurance, and under license medical professional. Family must not exceed maximum eligible family income as documented on IRS Tax Form 1040.

Provides medical grants to help with expenses for children, and gain access to health-related services not covered, or not fully covered, by their parents' commercial health insurance plan. Children must be 16 years old or younger while meeting other grant requirements.
