Children's Hearing Aid Program serves children 0-5 years old. Children ages 6+ may apply for the Hearing Aid Recycling Program. Qualifying children must:

• be a resident of Utah
• be diagnosed as having hearing loss by an audiologist
• provide documentation from an audiologist with pediatric expertise certifying that he/she needs hearing aids
• be ineligible to receive a hearing aid through the state’s Medicaid program
• not have insurance coverage for hearing aids
• meet the financial need qualification criteria established by the department for participation

Provides access to hearing aids for financially eligible families. Accepts donations of used hearing aids and unused batteries.

• Specialized Information and Referral

Individuals living with HIV disease and family members who have no insurance, are underinsured or are not eligible for a State program, assets not over $10,000 (does not include a home or one car), low income.

1. Supportive Services Program - ambulatory care, dental care, transportation services, case management, mental health services, substance abuse services, outreach services, and home health care.
2. ADAP (AIDS Drug Assistance Program) - helps eligible individuals pay for all or part of their HIV medications.
3. Health Insurance Continuation Program (COBRA) - can pay for COBRA payments for eligible individuals.

No restrictions

Provides immunization materials for the general public and health care providers, assess clinic immunization records, and collect immunization data through online reporting systems.

Provides legislative oversight, programs and training of professionals for newborn hearing screening in the state.

Provides comprehensive asthma care by ensuring availability of and access to guidelines-based medical management and pharmacotherapy for all Utahns with asthma, and to link those with poorly controlled asthma to individualized services. 

Services Include:

• Prescription Assistance Program for Patients with Asthma 
• Utah Asthma Home Visiting Program- Salt Lake and Utah counties, TriCounty and Southeast (Medicaid only)

Works to prevent sickness and death from infectious diseases and environmental hazards, and monitor diseases to reduce spread. The Bureau is also responsible for monitoring and responding to potential bioterrorism threats/events, communicable disease outbreaks, epidemics, and other unusual occurrences of illness. Among many programs, they run the Syringe Exchange program, a community-based program that provides access to sterile needles and syringes free of charge.

A health resource for Utah's rural, multicultural and underserved communities, the Office coordinates federal, state, and local efforts aimed at improving the health of Utah's rural, medically underserved, and multicultural residents. Works with communities that need assistance conducting needs assessments, recruiting health care professionals, grant writing, identifying sources of funding and implementing other projects related to decreasing disparity and increasing access to primary health care.

Autism, reside in Utah

Through the Autism Systems Development Program, the CSHCN Bureau provides leadership that brings together many of the agencies and advocates who influence the statewide system of services for children with autism spectrum disorder (ASD) and their families.

The program has four components: Utah Autism Initiative (UAI), Utah Registry of Autism and Developmental Disabilities (URADD), Data System Integration, and ASD Resource Management. The ASDP:
- Refers individuals with ASD to services for evaluation and treatment
- Refers to care coordination programs
- Updates ASD diagnostic providers list
- Updates ASD treatment resources list
- Expands previously conducted ASD services survey to assess service needs and identify
barriers for care across Utah
- Compiles annual report summarizing ASD needs in Utah

Statewide

0 - 18 years of age, children and adolescents with special health care needs in Utah. No fee.

Assists families of children and youth who have special health care needs with coordinated care planning, education and resources, in order for them to make informed decisions. This may include primary and special health care, behavioral health, developmental and educational programs, financial support resources and social services that meet their special needs from infancy through the transition to adulthood.

Services Include:

• Transition to Adulthood website provides age-specific recommendations and community resources for any Utah resident age 12-22 who requests assistance with transition preparation and readiness.
• Technology Loan Service- lends Chromebooks and hotspots to families of children with special health care needs. Available at locations in Blanding, Richfield, Vernal, Moab, Price, Ogden and Salt Lake City.
