0 - 18 years of age, children and adolescents with special health care needs in Utah. No fee.

Assists families of children and youth who have special health care needs with coordinated care planning, education and resources, in order for them to make informed decisions. This may include primary and special health care, behavioral health, developmental and educational programs, financial support resources and social services that meet their special needs from infancy through the transition to adulthood.

Services Include:

• Transition to Adulthood website provides age-specific recommendations and community resources for any Utah resident age 12-22 who requests assistance with transition preparation and readiness.
• Technology Loan Service- lends Chromebooks and hotspots to families of children with special health care needs. Available at locations in Blanding, Richfield, Vernal, Moab, Price, Ogden and Salt Lake City.

The Utah Department of Health's coronavirus information line and website (Spanish Website is also available, with Chat options as well) provides up-to-date information about the coronavirus, including prevention, preparedness, travel restrictions, vaccines, and response.For those experiencing symptoms or who are concerned about exposure to the virus, the information line is also available.COVID-19 Update/Testing: All state-sponsored (free) COVID-19 testing sites will close after March 31. This includes all Mako Medical, TestUtah, TourHealth, Utah Department of Health Mobile Testing Team, and the travel testing lanes at the Cannon Health building and St. George locations. People seeking COVID-19 travel testing will need to find a testing provider which offers the type of test required by their travel destination. People can find testing providers by contacting their travel agent, airline, or travel destination; or doing an online search.COVID-19 Update /Vaccination: As of June 21, 2022, all Utahns ages six (6) months of age and older are eligible to get vaccinated.
