Connector Service assists patients and families with scheduling and support services, including home visits by a registered nurse. Three or more chronic illnesses, such as: cerebral palsy, neurologic impairments, feeding issues, or vision/hearing impairment. A multi-disciplinary team assists children who have multiple, complex chronic illnesses. Interventions, including home visitation, help support the family so children can grow healthy in their home environments. Facilitating communication among providers is an essential objective. Shared Plan of Care.
