Eligibility:

Children and adults, a U.S. citizen or legal permanent resident, and a resident of the State of Utah. Eligible for services if they have qualifying disabilities in one of four areas: intellectual disabilities or related conditions; Acquired Brain Injury (ABI); physical disabilities; autism.

Services Provided:

Promotes opportunities and provides support for people with disabilities to lead self-determined lives. Support includes community living, day services, respite assistance, parental support, chore assistance, and support coordination.

Eligibility:

An applicant must be under 21 years of age at the time of admission; qualify for Medicaid based on the child's income and assets; meet admission criteria for nursing facility (NF) care; have at least one caregiver trained and available to provide care; require skilled nursing and/or rehabilitation services at least five days per week and be dependent on one or more of the following: a mechanical ventilator; tracheostomy-based respiratory support; continuous or bi-level positive airway pressure support (C-PAP or Bi-PAP); or intravenous administration of nutritional substances and/or medications through a central line; and choose to receive home and community-based services. A limited number of children may be served; child may qualify, but be placed on a waiting list.

Services Provided:

The Technology Dependent Waiver program provides the day-to-day administrative activities for Medicaid’s Home and Community-Based Waiver for Technology Dependent/Medically Fragile Individuals. The Centers of Medicare and Medicaid approve state waiver programs to offer an alternative to long-term care facilities by paying for supports and services to allow recipients to be cared for at home and in the community.

Eligibility:

Individuals must complete an application and meet income and other eligibility requirements. Special programs available for the blind, disabled, aged, pregnant woman, child under age 19, family with dependent children, women with breast or cervical cancer, and/or refugees.

Services Provided:

COVID-19 Update: Utah Medicaid members who are diagnosed COVID-19+ or in quarantine because of it can access transportation from hospital discharge to home, or from facility to facility by calling their local 911 and they will help make arrangements for the member. Medicaid is a source of health insurance coverage for Utah's vulnerable populations. Medicaid is a state/federal program that pays for medical services for low-income pregnant women, children, individuals who are elderly or have a disability, parents and women with breast or cervical cancer. To qualify, these individuals must meet income and other eligibility requirements.
