Services Provided: The Neurology Clinic, of which Muscular Dystrophy Clinic is part, accepts new patients on a referral basis only. Please have your referring physician's office fax the following information to our new patient schedulers: 1) a written referral outlining the neurological symptoms/diagnosis to be evaluated; 2) clinic notes and pertinent lab and radiology related to the diagnosis; and 3) a demographic outline: including the patient's permanent address, home and office telephone number, insurance information, and a social security number. Each case is evaluated by the proper physician.
