Eligibility: Utah residents, for people with disabilities with complex or acute needs, an optional Medicaid benefit.
Services Provided: A state operated Intermediate Care Facility, providing an array of resources and supports. Therapeutic, clinical and specialty services. Provides 24-hour-per-day services as well as a day program. Limited in-home respite services. 95% of the USDC is an adult population. The majority reside in our residential buildings. Clients each live in an apartment with four to six others. With staff help, they shop for groceries, cook their meals, and clean their living areas. Therapies include: speech services, nursing services, physical therapy, recreation services.

Services Provided: Neighborhood House offers home-like, nationally accredited toddler, preschool, afterschool, day camps and summer programs for children age 15 months to 12 years old as well as day care and support services for adults who need supervised care during the day. Clients receive the emotional, cognitive, physical, and social/spiritual stimulation needed to encourage independence, prevent isolation, and avoid premature placement in long-term care facilities.
